No, silly. You don't have to pay at all for this game.
En Masse and Blue Hole Studios were gifted an Unreal Engine 3 license (valued at over $700,000- http://devmaster.net/devdb/engines/unreal-engine-3) for free because of the great work they're doing in the medical field. Further, all the coding for this project was done for free by volunteers working round the clock to further science.
They hope to cure cancer with the research done watching players use the world of TERA, providing AI patterns for nano robots to go into the blood stream and fight off cancer and viruses. In fact, every BAM in the game has an AI script that is designed to closely mimic the movements of a malicious cell in the human body.
So not only do you play TERA for free, but you also play it for a good cause!
